ORDER FORM


There's plenty of options for placing an order. 
You may now download an order form, complete and return to us.  We can also fax or email you an order form, or you  may place your order by phone, e-mail, snail mail etc, or we can visit your office if you're located in our area. 

We're Currently Revising our Online Order Form and should have it ready shortly.

   
   
If you prefer to call, email or send a purchase order rather than the order form, here's a basic checklist of the information we need for most orders (don't hesitate to contact us if you have questions):
Your Contact info - Billing & Shipping Address, Phone, Email, PO number etc.

Item Description - Quantity, Catalog or Supplier Name and/or ASI Number, Item number, Color & Description, - Basically enough info to figure out which item you want.  If you found it on one of our supplier's sites, a link to the webpage would be very helpful.

Imprinting Info - Method of Imprint (if more than one option available), Color(s) of imprint, Ad Copy and any Special Instructions.  You can send camera-ready art or we can create artwork from your sketch or instructions (sometimes at no charge).

We'll promptly send you an acknowledgement or contact you if we have questions.
